Many LT companies have regional non-competes, so if you want to transition from LT to FT (not necessarily at the same facility where you were doing LT, just close enough to it) you may not be able to do that. One way around that is for your FT employer to buy out your LT contract, which costs a lot more than what I think people might think when they read "a cut". So one thing you should absolutely do when you are considering LT is understand exactly what the non-compete is, both in terms of geographic scope, and in terms of how it can be reversed. The situations where the LT company takes a cut are usually those where LT-to-FT was the original intent (similar to intern-to-hire, or contract-to-hire in other industries), rather than when you take on a job intended as LT and then decide you like it so much you negotiate it into an FT position. Also, if you are doing LT, you can pretty much forget about doing any kind of leadership or anything else big-picture in medicine. LT jobs are very much see-patient-treat-patient-repeat.
I am a Recruiter in the healthcare space. Locum tenens serve a purpose, relieving an understaffed or fluctuating staffing need. In my experience there are a few types of people that like Locum Tenen work. People who move around a lot, and the locum agencies help them stay employed and earning. Or people who have bumpy backgrounds that have trouble getting direct employment. Almost all of the agencies that provide locum tenens have a buy out clause, so an organization can pay a fee to directly hire the employee. Because we know this, we often assume (sometimes wrongly) that if you've been a locum for too long without the geographic reason, that there is a reason that you were not getting full time employment. This is inevitable where someone brings up that you can make more money as a locum. That's true the hourly rate tends to be higher than the corresponding full time rate. But, if they are using it to fill staffing needs, or handle a fluctuations then job security is much lower. You also tend not to get great benefits, if any at all.
